Facility

We have recently moved to a larger facility, conveniently located next to the Gem Theater in downtown Etowah.   We are now equipped with the most state-of-the art technology to provide the best care for our valued patients.  We are confident that you will feel right at home in our new office and welcome all patients as if they were family. 

Appointments

If you would like to make an appointment, please contact our office by phone or email. Please leave a message on voice mail if we are not available.  We make every effort to return every phone call within a timely manner. 

 

We understand that our patients schedules are very busy and emergencies do arise.  However, if you are unable to keep your reserved appointment with Dr. Pate, we kindly request at least 48 hours to reschedule.  We strive to accomodate every patient in need and we sincerely appreciate your cooperation. 

 
Financial

In order to provide the best care possible and to  keep our fees reasonable, we ask that you are prepared to pay for treatment on the day of your visit.   We offer payment plans through Care Credit.  Our office also accepts all major credit cards, check, debit cards and cash. Please call our office if you should have any questions.   Our team members can assist you in finding the best financial option for you and your family, allowing you to receive the dentistry you deserve.

    

Insurance

If you have dental  insurance, we are happy to assist you in filing your dental claim and assist you in maximizing your benefits.  Prior to treatment, we will provide you an "estimated patient portion" based on information given to us by your carrier.  However, your insurance carrier may change the amount they will pay at their discretion. 

Ultimately, you are responsible for Dr. Pate's entire fee, irregardless of what your insurance company pays.   If your carrier delays payment for more than 30 days, we kindly request your intervention.  If a claim is rejected or underpaid, we will do everything we can to assist you in filing an appeal for your claim.  However, we ask that you pay the balance of Dr.Pate's fee while the claim is in the appeals process.
